[EAPS 2026] Analytical Reliability of an AI-Powered Micro-Volume Digital Haematology Analyser across Infant, Paediatric, and Adult Specimens

Abstract
This study at Asan Medical Center compared miLab™ BCM, which runs an automated CBC and WBC differential from 5 µL of whole blood, with Sysmex XN in 100 infant, paediatric and adult specimens. Pearson correlations ranged from 0.94 to 1.00 for Hb, WBC, ANC and PLT, and mean bias did not differ significantly between age groups.
*Baseline = Sysmex XN reference mean (SD); Hb in g/dL, WBC/ANC/PLT in 10³/µL. r = Pearson correlation; mean bias = test device − standard device (Bland-Altman).
Key Highlights
- Method comparison at Asan Medical Center (Seoul) with 100 specimens: infants ≤1 yr (n=31), paediatrics 2–20 yr (n=35) and adults 20–64 yr (n=34).
- miLab™ BCM runs the CBC and WBC differential from a single 5 µL whole-blood sample in a sealed, single-use cartridge.
- Pearson correlation with Sysmex XN ranged from 0.94 to 1.00 across all age groups for Hb, WBC, ANC and PLT.
- Mean bias showed no statistically significant difference between adult and infant/paediatric groups (Dunn's test, all p>0.05).
- The larger PLT bias in infants reflects their higher baseline PLT levels rather than a true analytical difference.
- Specimen collection and reference testing were performed independently at Asan Medical Center.
